Senior Services – Social Worker – Freeborn County

JOB STATUS: Part-Time, 20-24 hours/week

LOCATION:  Freeborn County

MINIMUM EXPERIENCE:

Bachelor’s degree (B.A.) in Social Work or a related field from a four-year college or university.  One year of related experience and/or training preferred.

COMPENSATION:  $23.65-$26.61

Responsibilities include –

  • Providing information on respite care options to prevent caregiver burnout.
  • Linking caregivers to training, coaching, or support groups.
  • Helping a caregiver identify in-home supports for the care receiver.
  • Follow-up activities focused on the caregiver’s ability to sustain their role.
  • Providing options counseling to older adults about home-and-community-based services.
  • Assisting a senior in understanding Medicare or long-term care resources.
  • Connecting a senior to a chore, homemaker, transportation, housing, or benefits programs.
  • Follow-up activities focused on ensuring the seniors’ needs were addressed.
