Report Storm Damage Through Minnesota’s Individual Assistance Survey

If your home or personal property was damaged by recent storms, Minnesota Homeland Security and Emergency Management (HSEM) is asking residents to complete the Individual Assistance Survery Form 2026.

The survey helps state officials assess the extent of damage across Minnesota and determine whether additional disaster assistance may be available. Reporting your damage is an important step, even if you have insurance.

The survey is open now and will close at midnight on Monday, July 27, 2026.

Residents who experienced damage to their home or personal property are encouraged to complete the survey before the deadline.
